STATEMENT TO THE AUTHORIZATION FOR USE OF IMAGE AND/OR SOUND

I hereby grant Avaya Inc. its affiliates and subsidiaries ("Avaya"), the use of my image in photographs, video clips and / or the sound of my voice on audio tracks, in whatever format, singularly or in conjunction with other photographs, videos and / or sound recordings ("Collateral"), along with my name and title for advertising, publicity, commercial or other business purposes.

I agree the Collateral to be registered as intellectual property of Avaya, and that they can be reproduced or reused by Avaya in any media once accepted in this statement. I agree that I will not receive compensation, under any circumstances, for my participation. I also agree that the Collateral may be reproduced and presented by Avaya without liability, without need for revision or further consent from me, or any financial compensation.

I agree that my image and / or sound may be edited, copied, displayed, published or distributed in websites internal and / or external Avaya graphic material, or any other advertising medium at the discretion of Avaya.

Hereby, I release Avaya, its affiliates, directors, officers, contractors, grantees, representatives, agents and employees from any liability, claim or legal action which Avaya, its representatives or any other person acting on behalf of Avaya, have or may have as a result of this authorization. I will not present any claim or legal action against Avaya in relation to the rights granted in this permission, provided the use of the Collateral by Avaya is made in accordance with the rights granted herein.

I certify that I have full legal authority to declare under my name what the above refers to. It also shows that I have read the above statement, authorization and agreement, and confirm that I understand its legal scope.
